Presentation Overview:
The End-of-Life Pathway provides older Australians with up to $25,000 in home-based support over 12 weeks to remain at home during their final three months of life. It has been designed to complement the work of specialist palliative care services.
While this is a welcome addition to Support at Home, it comes with its own challenges in implementation.
I will share with the audience the eligibility criteria for using the pathway along with my experience of utilising it since its inception on November 1st, 2025.
