Biography:
Dr. Priyanka Vandersman is a Registered Nurse and Senior Research Fellow at Flinders University, Adelaide, Australia. Her research interests span palliative care, digital health, and aged care. She contributes to the national palliative care project, End of Life Directions for Aged Care (ELDAC), enhancing Australia's aged care sector's capacity in delivering quality end-of-life care through innovative approaches. Priyanka specializes in developing, implementing, and evaluating novel approaches to support end-of-life caregiving processes.
Abstract:
Background:
Digital transformation continues to reshape aged care, opening new possibilities for using data to strengthen quality, coordination, and end-of-life outcomes. With the Aged Care Act 2024, revised Quality Standards, and the Support at Home program now in effect, aged care providers are central to delivering integrated, person-centred care across both home and residential settings. While digital systems collect vast amounts of information, translating this into meaningful insight remains a challenge for many services.
This abstract describes a collaboration between a university research team and an aged care provider to embed an evidence based palliative care dashboard using a provider managed Power BI model. Designed to operate independently of commercial platforms, this model supports local adaptation, integration with existing workflows, and the meaningful use of routine data to guide care planning and improvement.
Method:
The project team partnered with XX[provider] to determine the most effective way to deliver this project. Through multiple rounds of joint consultation, the teams agreed on a three-stage approach as below:
1. Data Mapping: Identification and alignment of essential clinical/care data points across multiple management systems, with a flat-file option for providers using alternative platforms.
2. Dashboard Design: Development and validation a Power BI dashboard that converts routine data into practical insights for end-of-life care planning, reporting, and decision-making.
3. Provider Integration: Creation of an easy-to-follow implementation guide to help services with different levels of digital capability embed the tool into daily workflows and sustain its use.
Outcome:
This collaboration illustrates how research and technology partnerships can redesign end-of-life care by turning routine data into actionable knowledge. It demonstrates a practical and sustainable pathway for embedding evidence based, person centred end-of-life care into daily practice, supporting providers to navigate reform, strengthen digital capability, and improve care outcomes for older Australians.
